Accessing the Patient Portal

  • If you do not know your username and password, follow these steps on the portal page:
  • Select “Using Mobile Phone”
  • Enter your first name, last name, date of birth & hit submit
  • Select your mobile phone number and click “send code”
  • Enter the code and click “log-in”
  • If this is your first time logging in, you will be prompted to create a password and select security questions.
  • If your mobile phone number is not listed in our system, it will not appear when attempting to log in, in this case, please call 267-358-6076 for assistance.
