Patient Portal and Pay My Bill

Accessing the Patient Portal
- If you do not know your username and password, follow these steps on the portal page:
- Select “Using Mobile Phone”
- Enter your first name, last name, date of birth & hit submit
- Select your mobile phone number and click “send code”
- Enter the code and click “log-in”
- If this is your first time logging in, you will be prompted to create a password and select security questions.
- If your mobile phone number is not listed in our system, it will not appear when attempting to log in, in this case, please call 267-358-6076 for assistance.
