Glenohumeral Internal Rotation Deficit / Posterior Capsular Contracture

Glenohumeral Internal Rotation Deficit (GIRD) occurs as a result of a loss in the degree of shoulder internal rotation movement. This is determined by a measurement of a loss of 20° compared to the opposite shoulder.

 

Symptoms

  • Shoulder Stiffness

  • The feeling that a prolonged warm-up is needed

  • Loss of Velocity

  • Posterior Shoulder Pain

  • Pain in the late cocking phase of your throw

  • Posterior Shoulder Tightness

 

Causes

  • Repetitive overhead throwing

  • Rotator Cuff Tightness

  • Posterior Capsular Contracture

 

Diagnosis

  • Physician Exam

  • MRI

  • MR Arthrography

 

Prevention

It has been shown that stretching after a pitching session will help prevent symptoms of GIRD.

 

Treatment

Non-surgical:

  • Rest 

  • Ice

  • A prolonged course of Physical Therapy is usually recommended before any surgical intervention. This would include stretching and strengthening to improve shoulder mechanics.

 

Surgical:

  • Arthroscopic Shoulder Surgery
